Phone Appointment

Service Description

All appointments are by phone. Please be in a quiet environment and available during your scheduled time. A copy of your DD-214 is needed as proof of service and honorable discharge. After your initial certification, you will be scheduled for a ONE TIME 6 month follow up.

Cancellation Policy

Please notify us within 24 hours to cancel or reschedule your appointment. A $15 fee may apply. Your invoice with a link to pay your invoice will be in a separate email from this email.
