Advocacy Services

If you are seeking an Advocate for any reason, begin by completing this form to request an initial appointment.

Pregnancy Notification

Any clients that have become pregnant while their certification is active will need to complete this form to inform us immediately. Failure to do so will result in their certification being revoked.

If you have not received your card 15 days after ordering it, please email RA-DHMedMarijuana@pa.gov directly.
